Ingredients for Methadone for German Doctor

i need to find out the ingredients and uses etc. for my doctor in Germany, i'm going there for disc replacement surgery and he needs the info in German terms.i take methadone 1 10mg 4 x a day also the info for xanax 1 mg 4 x a day and trazodone 1 50 mg as needed for sleep can you help me with this info? mainly the methadone..or where can i get this info to send to Germany?
