How do i know if my clots have gone

I have been on warfarin for 5 months after a pe during pregnancy or my c. section they think? They want to take me off in sep i am afraid so afraid it will happen again. How do i know my clots have gone as doctors don't check this, i feel like a ticking time bomb and that i could die any time. I was in a mother and baby unit for 3 months with a severe anxiety disorder and depression due to this please help i live in fear and doctors wont listen. before they even found my clots they sent me away 3 times saying i was out of breath because i was anxious, then they did a ct scan saying it was fine then called me in a week later to say they had found a small clot, then all of a sudden it was 2 then lots of little ones.
